1) Describe the purpose of physical therapy annual visits for aging adults.
2) Use the Annual Physical Therapy Visit for the Aging Population template and tools
3) Interpret results of a physical therapy annual visit and make decisions about next steps including referrals to appropriate healthcare providers and community resources.
4) Develop a model for implementation of the annual visit within their current practice setting.
About The Speaker(s)
Gina Pariser, PT, PhD, LDE
Gina Pariser, PT, PhD, LDE is an educator and teaches courses on
the application of exercise physiology, health promotion and wellness, and
research in physical therapy practice. Her work as a physical therapist focuses
on management of chronic health conditions. She developed an interprofessional
service learning clinic called Active Steps for Diabetes to enrich the student
learning experience and strengthen the communities served by the program. Her
scholarship focuses on health promotion programs to improve mobility in
medically underserved populations with chronic disease. She is an active member
of the American Physical Therapy Association including leadership roles in the
Academy of Geriatric Physical Therapy.
Michael Puthoff, PT, PhD
Dr. Puthoff is a Professor in the St. Ambrose University Physical Therapy Department and is currently serving as Program Director. He completed his physical therapy training at Marquette University and received his PhD in Physical Rehabilitation Science at the University of Iowa. He teaches courses in exercise physiology, health and wellness, cardiopulmonary therapeutics, and the management of aging adults. His scholarship interests are the implementation of physical fitness screens with aging adults and physical therapy management of individuals with venous thromboembolism. He also has interests in incorporating principles of Lifestyle Medicine into physical therapy practice. Dr. Puthoff is clinically active in the skilled nursing facility setting and services as a faculty for the St. Ambrose University Interprofessional Health Clinic.
Hadiya Green Guerrero, PT, DPT
Hadiya Green, PT, PDT is an equity-lensed, community integrator who specialized in movement. She attained her Bachelor's in Sociology from Princeton University, a Doctor of Physical Therapy degree from Washington University in St. Louis, and is a board-certified specialist in sports physical therapy. She began coaching collegiate and high school sports and personal training adults of all ages in 1998, in addition to beginning her work in disparity-related research from prostate cancer to children with ADHD and later treatment and function around musculoskeletal conditions. Dr. Green served six years as senior practice specialist at the American Physical Therapy Association and as a physical therapist, instructor, and principal investigator at Mayo Clinic. She's practiced physical therapy over 18 years with clients, spanning all ages and including populations from Wall Street, Harlem NYC, Mayo Clinic - Rochester, and Washington, DC. As founder of Healthy Healing Community, a subsidiary of Hadiya Green, LLC, and movement expert she addressees well-being through education, consulting, and whole-person and population intervention via injury prevention, health promotion and wellness.
Devon Hock, PT, DPT, GCS
Devon Hock, PT, DPT is a clinical liaison to the Best Practice workgroup of the Health Promotion and Wellness SIG with specialty practice areas in dementia and fall prevention for which her research and programs have been selected for presentation within the National PACE Association. She currently works at a skilled nursing facility as well as Rehab Essentials as a Capstone Advisor for transitional DPT students. Devon also sits as a Commissioner on the Council of Aging for the town in which she resides.
David W.M. Taylor, PT, DPT, CGS
David W.M. Taylor is a Clinical Associate Professor and Director of Clinical Education in the Department of Physical Therapy at Mercer University. He received his undergraduate and professional physical therapy education at Emory University. He is a Board-certified Clinical Specialist in Geriatric Physical Therapy, and Distinguished Fellow of the National Academies of Practice. Dr. Taylor has practiced clinically and in academia for over 30 years in a variety of settings, across the continuum of care, with an emphasis on aging adults and fall prevention. He is a member of ENGAGE, which is part of Georgia GEAR, a multi-institute partnership whose goal is to improve clinical care and quality of life for older adults and their families in Georgia.
